Derby City, Nottingham City and Nottinghamshire County (DN2) have commissioned Outcomes for Children to develop a new and innovative service, STARS (Supporting Children and Young People to Achieve Resilience and Success).  Working across the three Local Authority boundaries, STARS will support children and young people in local authority care and on the edge of care, to achieve better outcomes.  STARS is delivered by Polaris Children’s Services on behalf of Outcomes for Children.

We support children and young people to:

  • Transition from residential care to foster care placements; 

  • Be supported to avoid escalation from fostering to residential care; 

  • Support children and young people to be re-unified with their families/primary carers; and, 

  • Help children and young people to remain out of care.

What we do

Uniquely, STARS provide support to children, young people and their families over a two year period.

 

STARS use a flexible, multi-disciplinary ‘one team’ therapeutic and solution focused delivery model to enable children and young people to achieve stable placements.

 

Our team consists of Social Workers, Family Support Workers, Education Officers, Therapists and Care Experienced Peer Mentors.

 

Our mission is to support children and young people to achieve resilience and success through working in partnership with them, their families and professionals to provide tailored, intensive, and focused support that achieves positive and demonstrable outcomes, which are significant and sustained.
